Your Right to Receive a Good Faith Estimate

Under the No Surprises Act, health care providers are required to provide individuals who do not have insurance or who choose not to use their insurance with an estimate of the expected costs for medical and mental health services.

At Heritage Path Counseling, we believe that understanding the cost of care is an important part of making informed decisions about your treatment.

What Is a Good Faith Estimate?

A Good Faith Estimate (GFE) explains the expected charges for the mental health services you receive.

The estimate includes the anticipated costs of services that are reasonably expected for your care, including therapy sessions and any related services provided by Heritage Path Counseling.

Please note that the Good Faith Estimate is an estimate only. Actual charges may vary depending on your treatment needs, the number of sessions, or changes in your treatment plan.

Who Is Eligible?

You have the right to receive a Good Faith Estimate if you:

– Do not have health insurance.
– Choose not to use your health insurance for mental health services.
– Are paying for services yourself (self-pay).

When Will I Receive My Estimate?

You have the right to receive a Good Faith Estimate before your scheduled services.

Generally:

– If you schedule an appointment at least three business days in advance, you should receive your estimate within one business day after scheduling.
– If you schedule an appointment at least ten business days in advance, you should receive your estimate within three business days after scheduling.
– You may also request a Good Faith Estimate before scheduling services.

What Information Is Included?

Your Good Faith Estimate may include:

– The expected cost of your initial evaluation
– The estimated cost of ongoing therapy sessions
– Any additional services reasonably expected as part of your care
– Provider information
– Practice contact information

If Your Costs Change

Mental health treatment is individualized. Your treatment plan may change over time based on your needs, goals, and progress.

If there are significant changes that affect the expected cost of your care, Heritage Path Counseling will provide an updated Good Faith Estimate whenever appropriate.

If Your Bill Is Much Higher Than Your Estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the patient-provider dispute resolution process established by the U.S. Department of Health and Human Services (HHS).
